only when there's visible milk, poop, or puke. and i don't actually clean the yucky one...i put a new one on, and the old one sits on the basement floor for a couple of months, until i get inspired to wash it.

There is nothing better than clean sheets.
p.s. I am on day 9 of coffee detox. I am officially on tea and lemon water.
